Intake Evaluation

During the intake evaluation, we will get to know you and learn about the concerns that prompted your visit.  We will also ask a series of diagnostic questions so that we can clarify diagnosis and identify the most appropriate treatment for your care. We will also begin to identify goals for treatment.

Individual Therapy

55 minutes, $160

45 minutes, $150

Individual therapy sessions will utilize evidence based treatments to help move you in the direction of your values and goals.  We typically begin meeting 1x/week and space out sessions as treatment progresses.

Training provider:

$50 return session (45-55 minutes)

When seeing a training provider who is working under the supervision of an attending-level provider, we offer a reduced self-pay rate of $50.  Please note that these sessions cannot be filed to insurance or submitted for reimbursement.

A Few Notes About Insurance

• Flourish Mental Health, PLLC is in-network with Blue Cross Blue Shield (BCBS).  This includes the Blue Cross State Health Plan and PPO Plans (it does not include Blue Home or Blue Value).  Flourish is a part of the Clear Pricing Project with the BCBS State Health Plan, so the co-pay is $0!

• If we are not in-network with your insurance, that doesn't mean we can't work together!  Many insurance companies have out-of-network coverage for mental health services that are equal to or slightly less than in-network coverage.  As a courtesy, we will file the necessary information with your insurance company on your behalf and they will subsequently provide reimbursement (based on your out-of-network coverage).

  • Note: the one exception to this is United Healthcare.  We are still happy to see United Healthcare clients.  However, we have had difficulty submitting claims on behalf of clients to United Healthcare and want to be sure that this process is smooth for clients.  Instead, we are happy to provide a "super bill" for individuals who have United Healthcare and want to use out-of-network benefits.  I am told by clients that submitting the claims is very easy and fast.

• Questions to ask your insurance company include:

  • Is Flourish Mental Health, PLLC in-network?

  • What is my coverage for a 90791 (intake evaluation)?

  • What is my coverage for a 90837 (53+ minute therapy session)?

  • What is my out-of-network coverage?

• In keeping with the No Surprise Act, individuals who do not have insurance or have insurance and do not plan to file for coverage, are entitled to a Good Faith Estimate.  The Good Faith Estimate is an estimate of the cost of services for one year and will be provided to you before treatment begins.  Additionally, if services exceed that estimate by $400, you have the right to dispute them.
